He is survived by: his parents, Wade Polly (Donna) of Austin, Indiana and Melissa Hilbert (Bill) of Scottsburg, Indiana; and his sisters, Jessica Polly of Scottsburg, Indiana and Linda Polly of North Vernon. He is also survived by 7 nieces and nephews.
