He is survived by: his significant other Jamie Rollins of Seymour; his sons, Dustin Chumley of Bedford, Indiana, Devin Chumley (Annie) of Princeton, Indiana and Dillin Chumley of Bedford; his siblings, Thomas Chumley Jr. (Wanda) of Ratcliffe, Kentucky, Mack Banks Jr. of Bedford, Danny Banks of Bedford, Davey Banks of Bedford, Cathy Brown (Robert) of Bedford, Cheryl Chumley of Stafford and Virginia; and his grandchildren, Aiden, Kenner, Gracie and Paisley.
