He was predeceased by: his parents, Alvin Smith and Pauline Smith; his sisters, Evelyn Followell and Lisa Krahn; and his nephew Patrick Followell.
He is survived by: his wife Cindy Black of Seymour, IN; his daughters, Shannon Longbrake (Jeremy) of Seymour, IN and Heidi Kerns (Ryan) of Indianapolis, IN; his grandchildren, Heather Altepeter, Kylie Fourman (Matt), Jamie Longbrake, Jocelyn Reynolds and Elena Cooksey; his great grandchildren, Gracelynn, Gabrielle, Taygen, Scarlett and Eleanor; his siblings, Alice Dooley (Dale) of Madison, IN and David Black of Florida; and his nephews, Matt Followell and Eric Raes.
A graveside service was held on Wednesday, October 15th 2025 at 2:00 PM at the Riverview Cemetery (1603 Shields Ave, Seymour, IN 47274).
