She is survived by: her parents, Martin Eberhart and Margaret Eberhart (Quinton) of Shelbyville; her husband Gary Sample; her siblings, Duane Eberhart (Thomas) of Edinburgh, Jennifer Clark (Christopher) of Edinburgh and Sarah Eberhart (Amanda) of Shelbyville; and her nieces and nephews, Morgan, Maislyn, Elias and Quinton.

In lieu of flowers, memorial contributions may be made to the Shelbyville-Shelby County Animal Shelter, 705 Hale Road, Shelbyville, Indiana 46176.
