He was predeceased by: his siblings, David Forman, Roger Forman and Jerinda Forman. He is survived by: his siblings, Gerry Ann Kalb of Shoals, IN, Carol Forman of Lafollette, TN and Daniel Forman; and his half-brother William Keffer (West Virginia).
