He was predeceased by: his parents, Oscar Gene and Phyllis Church (Divens); and his brothers, Ronnie and Adam. He is survived by: his wife Loretta Lynn Roach; his children, Daphney Freeze (Steve Sr.) of South Bend, Jennifer Janiak (John, Sr.) of South Bend, David Gene Church Jr. of South Bend and Holly Estrada (Hector) of Orlando, Florida.; his grandsons, John Janiak Jr., Jacob Janiak, Dalton Church, Steven Freeze Jr. and Brandon Freeze; and his siblings, Tina Kimberlin and Danny Church of Mishawaka. He is also survived by many nieces and nephews.
