He was predeceased by: his father Edward Chwalek; and his stepson Derrick Mills. He is survived by: his spouse Rozalia Vida; his children, Edward Chwalek (Lori) of South Bend and Nicole Evans (Rick) of South Bend; his stepsons, Shane Flowers of South Bend, Dustin Mills of Indianapolis and David Mills (Elisha) of Walkerton; his grandchildren, Marisa, Alexis, Ashley, Mckenzie, Austin, Nick, Dillan, Aleah, David Jr., Leanne, Adrianna, Dakota, Braylee and Zoey; his great grandchild Kohlston; and his siblings, Michelle Vascil (Steve), Alex (Debora), Zolie (Gail) and Tezera (Eric). He is also survived by several nieces and nephews.
