She was predeceased by: her parents, Bert A. Kozlowski and Julia V. Kozlowski (Rojics); her spouse Sidney Moore, Jr.; her stepdaughter Christina Williams; and her siblings, Marie Lambert and Robert N. Kozlowski.
She is survived by: her son Rick A. Modlin (Cherie A. Bittle) of Niles; her grandchildren, Ryan A. Modlin of Niles and Brittany D. Williams (Jesse) of Niles; her stepdaughters, Cynthia K. Granlee of Bakersfield, CA and Connie M. Porter (Tom) of Tubac, AZ; her siblings, Julie Vance of Brevort, Bert A. Kozlowski of Eugene, Carl A. Kozlowski of Mishawaka and Michael D. Kozlowski of South Bend; and her son Todd Sheperd (Lori Myers-Weilers) of South Bend. She is also survived by 14 step-grandchildren and 25 step-great grandchildren.
