She was predeceased by: her parents, William Aftowski and Stella Aftowski (Michalski); her husband Henry L. Polak; her son Richard Polak; and her sister Mary Ann Widup.
She is survived by: her daughter Kathy Watts (Charles) of Johns Island; her grandchildren, Karmin Bowers Meade (Jason) of Johns Island, Jennifer Watts Veldman (Wayne) of South Bend and Jeremy Watts of South Bend; and her great grandchildren, BriAnna Watts, Elizabeth Meade, Graham Meade, Lydia Meade, Austin Veldman, Colin Veldman, Carter Veldman, Charles Watts, Theodore Veldman, Daniel Veldman and Kate Veldman.
