He was predeceased by: his parents, Richard Leon Ellsworth and Rosalyn Carol "Rose" Ellsworth (Sherow). He is survived by: his wife Brooke Westphal; his child Chase Ellsworth; his sister Robin Kamp (Ellsworth); and his nieces and nephews, Haley Ellsworth, Richard Kamp and Riley Hughey (Brian). He is also survived by many extended family members, friends.
