He was predeceased by: his parents, Harry Michalak and Lillian Michalak (Kush); and his sons, Randall and Andrew Michalak. He is survived by: his wife Patricia Anne Michalak (Wesolowski); his son William; his siblings, Bob Michalak (Fran) and Ann Horn (Tim); and his grandson Jett.
