He is survived by: his spouse Gail Fortner (Chalk); his children, Thomas Michael Fortner (Nicole) of Brandenburg, KY and Erin Nicole Fortner; his siblings, Danette Clift (Richard) of Pendleton, KY, Berney Fortner (Sandy) of Huntington, WV and Monté Mallory (Rebecca) of Moores Hill, IN; and his grandsons, Adam James Fortner and Liam Thomas Fortner.
