He was predeceased by: his parents, Hubert Laverne and Lucy Johnson (Hook); and his sisters, Donna Long, Diana Hlatko and Connie Dickerson.

He is survived by: his significant other Cyndy; his sons, Matt (Nathalie) and Sam (Heather); his grandchildren, Camden, Sierra, Corbin, Jade, Kailer and Wyatt Johnson; and his great grandchild Rowan Johnson. He is also survived by several other family members and friends.
