He was predeceased by: his parents, Richard Alfred Mayrose and Anna Mildred Smith Mayrose. He is survived by: his wife Cara Wilson Mayrose; his children, Carly Joanna Mayrose and Dale Richard Mayrose (Kim); his grandchildren, Alyssa Carr Mayrose, Adam Charles Mayrose and Robert Wayne Mayrose (Kristy); his great grandchildren, Titus Richard Mayrose and Samantha Louise Mayrose; his sister Louise Mayrose Cadick; his nephew Ross Joseph Cadick (Amber); his great-nephews, Benjamin Thomas Cadick and Clark Richard Cadick; his niece Anna Malinda Cadick Coyne (David); his great-niece Eleanor Louise Coyne; his great-nephews, David Alex Coyne and Carson Wayne Coyne; his cousin Dale Hudson; and his father-in-law Joseph Wilson. He is also survived by his extended family and cared for many of the cousins.
In lieu of flowers, the family requests that contributions be made to the Richard Mayrose, M.D., Memorial Scholarship Fund at Union Health Foundation.
