He was predeceased by: his parents, Joseph A. Koontz and Opal G. Koontz (Baugh); and his siblings, David, Gary, John Koontz, Janet Koontz Lanpher and Donna Koontz Hankins.

He is survived by: his wife Carolyn Koontz; his daughter Jennifer Scott (Dave); his grandchildren, Drake Koontz (Danny), Drew Fields and Jada Scott; his great grandchildren, Jack and Michael Koontz; his stepgrandchildren, Marla Hyman (Aries), Paige Beard (Tom) and Dawson Scott; his stepgreat-grandchildren, Hadley Fuller, Blake Scott, Bohdi and Barrett Beard; and his brother Steven Koontz (Shelley). He is also survived by several nieces and nephews.
