He was predeceased by: his parents, George Cook and Blanche Cook (Nevil); his children, Christopher DeWayne Cook and Dianna Lynn Studebaker; and his siblings, James, George Fred, Richard Cook and Peggy Marlene Bowman.
He is survived by: his wife Carol Ann Cook (Rosswurm) of Uniondale, IN; his children, Kevin L. Cook (Jodi) of Monroe, IN and Raquel Ann Link (Lloyd) of Craigville, IN; Marvin Cook (Rochelle) of Geneva, IN.. He is also survived by 10 Grandchildren, 14 Great Grandchildren, 2 Great Great Grandchildren, 5 Step Great Grandchildren.
