He is survived by: his mother Joyce Horn (Patrick) of Madison; his daughter Tiffany Horn of Osgood; his pet Tiffany Horn of Osgood; his siblings, Jeff Horn (Tanya) of Holton, Daniel Horn (Lisa) of Batesville, Melissa Manlief (Dwight) of West Port and Catina Kaiser (Robert) of Madison; and his daughter Patricia Holman (David) of Osgood. He is also survived by many nieces and nephews.

Memorials may be given in honor of Doug to the American Diabetes Association.
