He is survived by: his wife Lisa Cook of Wabash; his sons, Michael Cook (Jourden) and Marcus Cook (Jullian Hart) of Marion; his stepson Zachary France of Indianapolis; his granddaughter Madison Cook; his mother-in-law Judy Hannah of Wabash; his brother Rusty Cook of Arizona; his siblings-in-law, Lori Stewart (Brian "Poco") of Chattanooga and Michael Elliott (Theresa) of Wabash; and his nieces and nephews, Evren Elliott and Zoe Elliott. He is also survived by several aunts, uncles, and cousins.
