He is survived by: his wife Shaunita Pugh; his daughters, Carrie Ennis (Ben), Shae Smith (Adam) and Mariah Polen (Nick); his sister Nancy Leslie; his grandchildren, Gabrielae Fred, Aleck Fred, Summer Johnson, Stormi Smith and Miles Polen; his mother-in-law Delores Pugh; his nephew Trevor Lesley (Mandy); and his niece Shanan Swartz (Natasha).
