He was predeceased by: his parents, Rufus Lee and Regency Perkins (Hook); his son Stephen Ray Perkins; and his sister Martha Sebestian. He is survived by: his children, Stacy Collins (Ron) of North Vernon and Scott Perkins (Annette) of Westport; and his stepdaughters, Dana Rouse of North Carolina and Dina Sherrill (Tony) of North Vernon.

Memorial contributions may be made through the funeral home to the Sardinia Baptist Church, the Westport Volunteer Fire Department or Our Hospice.
