He was predeceased by: his parents, Alvin Cook and Betty Cook (Thorp); his wife Brenda Evans; his brother Al Cook; and his daughter-in-law Debbie Cook.
He is survived by: his children, Chris Cook, Philip Cook (Lisa) and Tommie Ott (Tony); and his siblings, Linda Fischer, Dorothy Sanders, Keith Cook, Denny Cook and Mike Cook. He is also survived by 5 grandchildren, 10 great-grandchildren.
