He was predeceased by: his parents, Richard Cook and Valeria Cook (Sutton); his wife Julie Cook; his brother Darran Cook; his father-in-law Newt Ward; and his nephews, Hayden Cook and Conner Ward.
He is survived by: his children, Jake Cook (Angela Cook) and Amanda Gorges (Shane Gorges); his grandchildren, Noah Gorges, Emmarie Cook, Noah Roark, Chase Cook, Jacob Cook and Magdalen Cook; his mother-in-law Diane Ward; his siblings-in-law, Darren Ward, Jill Ward and Susan Cook; his nephew Harrison Cook; and his nieces, Sydney Cook, Ashley Ward and Emily Ward. He is also survived by his children and grandchildren.
