He is survived by: his former wife Barbara Middleton (Nelson); his children, Brice Middleton (Gretchen) and Amy Miles; his stepchildren, Bret Poulos (Kelli) and Julie Booker (Jeff); his grandchildren, William, Matthew Middleton, Jordan Booker (Robin), Joshua Booker (Kayla), Ian and Hayli Poulos; and his great grandchildren, Jailee and Jayden.
