He is survived by: his mother Shirley Marie Miller; his wife Stephanie Miller; his daughter Faith Miller; his stepson James Blauvelt; his siblings, Carol Hayes (Dana), Michelle Borden (Ron) and Paul Hayes; and his cousin Chris Williams (Stephanie).
