He was predeceased by : his parents, Henry Cook and Alice Cook (Esson). He is survived by : his wife Peggy Ballinger; and his children, Mike Cook, Daniel Cook, John Ballinger and Jesicca Leapaldt. He is also survived by grandchildren.

A celebration of life was held on Thursday, April 25th 2024 from 1:30 PM to 2:00 PM at the Fellowship Christian Church (614 Cedar St, Concordia, KS 66901).
