He is survived by : his children, David Jr. The (Rachel) of Olathe, Denise Mulich of Kansas City, Missouri and Jennifer Phillips of Kansas City; his grandchildren, Keilan, Kingsten and Maicie; his daughter Amy Sago (Greg) of Basehor; and his grandchild Kai.
