He was predeceased by: his parents, Paul Wayne Cook, Sr. and Ruth Thelma Younger; and his siblings, Paul Wayne Cook, Jr., Michael Frank Cook and Anita Katherine Frost.

He is survived by: his children, James Cook, Jr. (Teresa), Johnny Cook (Kelly), Warren Mathers (Mary), Christine Mathers, Linda Pence (Skip), Carol Chaffin and Crystal Gould (Eli); and his brother-in-law Don Frost. He is also survived by seventeen grandchildren; thirty-seven grandchildren.
