He is survived by: his wife Marilyn Kostusik; his children, Kara Smith, Kurt Uniak (Leanna) and Korie Johnson (Scott); and his grandchildren, Cooper, Catie Smith, Kade, Lydia Uniak, Madisyn, Bennett and Baylor Johnson.
