He was predeceased by: his parents, Willard Dean Sr. and Willa Hazelitt (Lacy); and his siblings, Anthony and Ruth Antoinette. He is survived by: his former wife Genevieve Lurquin-Hazelitt of Leavenworth, KS; his son Willard "Bill" D. Hazelitt III; his sisters, Theresa Tilton of Highland, KS and Deborah Foley of Bonner Springs, KS; and his granddaughter Anna-Jean Marie Hazelitt. He is also survived by several Nieces and Nephews.
In lieu of flowers the family suggests memorial donations in his name to the Parkinson's Foundation.
