He was predeceased by: his parents, Elmer "Tim" Hettinger and Jeanette Hettinger; and his cousin Billy Garner. He is survived by: his wife Terry Carmack; his children, Chris (Allison), Michael, Jr. and Rachel (Kyle Nash); his grandchildren, Kate, Jack, Christian, Cooper and Jordan; and his caregiver Dr. Jonathan Jacobs.
