He was predeceased by: his parents, Waldo Cook and Nora Cook; his son Brad; and his siblings, Archie, Wayne, Bobby, Darlene and Sharon.
He is survived by: his wife Frances "Jean" Cook; and his grandchildren, Derik Cook (Jessica), Joshua Swanson (Tristyn), Tiffany McIntosh and Jenna Cook of Springfield, MO. He is also survived by six great-grandchildren.
