She is survived by: her children, Barbra Levine Pakravan (Pak), Jack Levine (Kristen) and Randy Levine (Bob Presz) of Milwaukee; and her sister Barbara Rose of Sonoma, California..
In lieu of flowers, the family prefers donations to be made in memory of Jan Levine to Hospice of Siouxland.
