He is survived by: his children, Jaci Hazen of Ottawa, KS, Steve Smith (Marianne) and Jeff Smith (Dawn); his grandchildren, Tyler Hazen (Dana), Adam Hazen, Abby Chappell, Emily Smith and Kayla Smith; his stepgrandsons, Dillon Boeh (Celsie) of Ottawa, KS and Drew Boeh of Ottawa, KS; and his siblings, Audrey Gales of Fort Dodge, IA, Donna McCullough (Jerry), Meg Staudt and Randy Holmes (Carol). He is also survived by many nieces and nephews.
