He was predeceased by: his father Jerry L. Cook; and his grandparents, Grant, Betty Cook, Farroll "Hap" and Helen Clark.
He is survived by: his wife Gina Sreaves; his daughter Tess Cook of Lee's Summit, Missouri; his mother Gayle Cook of Butler, Missouri; his brother Brian Cook (Dana) of Leawood; his nieces and nephews, Brenna McGuire (Austen) and Bailey; his brother Pirapon Namatra (Fon) of Bangkok; his nieces and nephews, Yim, Kiki, Itt and Toh; his sister Lilana Peck of Olathe; his nieces and nephews, Ava and Ander; his brother-in-law Jess Sreaves (Kathy) of Fairview; and his child Lindsay. He is also survived by many aunts, uncles, and cousins.
In lieu of flowers, donations can be made to Heart of America Alzheimer's Association.
