He is survived by: his significant other Anita (family: Mark Chaplick, Major Craig Chaplick, Melanie Fletcher, Mindy Riley, Andrea Price, Stephen Chaplick); and his caregivers, Peggy, Rachel, Kim and Autumn. He is also survived by grandchildren and great-grandchildren.
