He was predeceased by: his parents, John Edward and Viola Mae Middleton (Walls); his children, Rodney Dean Middleton and Kim Denise Middleton Orr; and his siblings, Robert "Bob" Middleton, John Middleton, David Middleton and Irene Fender.

He is survived by: his wife Phyllis Middleton; his children, Mark Alan Middleton (Natasha) of Olathe and Diane Michelle Middleton Davis (Douglas) of Parsons; his stepchildren, Dean Douglas Garland (Dianna) of Overland Park and Gina Kaye Garland of Parsons; and his grandchildren, Zachary Middleton, Kierstin DeNoon (Ashton), Connor Middleton (Michaela), Lucas Middleton (Anna), Mark Stepanov, Matthew Middleton, Keri Davis, Jeffrey Garland (Tara), Amanda Garland, Calvin Garland (Gretchen), Tyler Garland, Nolan Webster and Paiton Webster (Clayton). He is also survived by 10 great-grandchildren and one on the way, Numerous nieces, nephews, and cousins.
