He was predeceased by : his parents, Allen Hammack and Rutheen Hammack (Metcalf); his former life partner Margaret Forshey; and his sisters, Mary Alene Pontious and Carol Ann Martin.

He is survived by : his children, James "Jimmy" Hammack (Tammy) of Carthage, MO, David Hammack (Janet) of Griffithville, AR, Shawn Hammack (Courtney) of Almyra, AR, Clint Hammack of Parsons, Tina King (Gaylon) of Searcy, AR and Shannon Stadola (Joe) of Mound Valley; his close friend Freddie Rittenhouse; his brothers, Roger Hammack (Vicki) of Elk City, Fred Hammack (Angie) of Altamont and Bobby Hammack of Pawhuska, OK; and his aunt Londa Mae Metcalf of Parsons. He is also survived by 11 grandchildren; five great-grandchildren.
