He was predeceased by: his parents, Reverend Buddy William Forehand and Jettie Louise Forehand (Ellers); his wife Patricia Anne Forehand (Bendt); and his siblings-in-law, Tinna Marie Bendt and William Henry Bendt III.
He is survived by: his brother Thomas M. Forehand of Joplin, Missouri; and his siblings-in-law, Kenneth W. Bendt of Girard, Kansas, Nancy L. Bendt of Pittsburg and Nishua F. Phillips (David R. Phillips) of Republic, Missouri. He is also survived by several nieces, nephews, great-nieces, great-nephews and great-great-nephews.
