He was predeceased by: his parents, Thomas Forge and Agnes Forge (Doman); and his siblings, Joan, Agnes and Cyril.
He is survived by: his wife Angela Fink; his children, Melinda Forge of Topeka and Dale "Dee" Forge Jr. (Janet) of St. George; his grandchildren, Emily Lynch (Alexis) of Lane Forge, DVM, Rachel Forge and Skyler Forge (Katie Willard); his great grandchildren, Presley Lynch and Brooks Lynch; and his siblings, Tom Forge Jr. and Audrey Fitzgerald.
