He is survived by: his wife Charlene Boyd Qualls of Ashland; his children, Michael Curtis Qualls II (Dana) of Greenup, Kentucky and Stephanie Qualls Jordan (Spanky) of Ashland; his siblings, Timothy Brian Qualls (Linda) of Pedro, Ohio, Carmen "Sis" Marie Hankins (Anthony) of Ashland and Kimberly Jean Jones (Billie) of Kitts Hill, Ohio.; his granddaughters, Carlie Knipp (Robert) of Ashland and Cambrie Jordan of Ashland; his great grandchildren, Owen Waugh and Lincoln Michael Knipp; and his pet Odie.
In lieu of flowers, memorial contributions may be made to CornerStone Methodist Church, Community Hospice, or the American Red Cross.
