He was predeceased by: his brother Anthony Ballard. He is survived by: his mother Nora Marie Downs Ballard of Bardstown; his wife Carol Lyvers Ballard; his daughters, Nikki Parkerson (Wesley) of Bardstown, Kaitlyn Ballard of Bardstown and Clara Ballard of Bardstown; his father Joe Ballard; and his siblings, Karen Harmon (Jody) of Bardstown, Donna Smith (Robert) of Bardstown, Wayne Ballard (Rhonda) of St. Francis and Pat Ballard (Katrina) of Bardstown.
Expressions of sympathy may take the form of contribution towards funeral expenses.
