He was predeceased by: his parents, Jack Winston Hazelwood and Grace Rose Hazelwood Huff. He is survived by: his wife Debbie Cain Hazelwood; his daughter Nicole Heckel (Grant); his siblings, Jack Hazelwood Jr., Steven Hazelwood and Lori Renner (Dennis); and his granddaughter Adeline Heckel.
Visitation will be held on Friday, May 15th 2026 at 12:00 PM at the Reppert Funeral Home (110 N Broadway, Berea, KY 40403). A funeral service will be held on Friday, May 15th 2026 at 2:00 PM at the same location.
