She was predeceased by: her parents, Edward Kozuszek and Wilma Kozuszek of Washington Park, IL.; her significant other Roy; and her brother Ronald Kozuszek (Roy). She is survived by: her daughter Amy Smith (Mark) of Alvaton, KY.; and her granddaughters, Natalie, Lauren Smith and Amelia Hackworth.
