He was predeceased by: his grandparents, Glen and Jehree Britt; and his aunt Jerri Britt. He is survived by: his parents, Rick Cook and Sherri Cook of Scottsville, KY; and his brother Matt Cook (Breanna) of Bowling Green, KY.
In lieu of flowers, the family requests that donations be made to the American Cancer Society or St. Jude's. Donations may be made at Goad Funeral Home.
