He was predeceased by: his parents, James Yates and Goldie Yates. He is survived by: his wife Carol Yates; his children, Jamie Washer (Van), Jim Yates (Tracey), David O'Donnell (Jenny) and Jennifer Lawson (Kyle); and his siblings, Donnie Yates (Loretta) and Ann Casteel. He is also survived by 9 grandchildren; 6 great grandchildren.
In lieu of flowers, donations may be made in Jay's memory to Hospice of Southern Kentucky.
