He was predeceased by: his parents, Roy Lewis and Ethel Graham Lewis House; his grandson Nathan Lewis; his uncle Gus; his aunt Lois Norris; and his siblings, Roy Thomas Lewis, Virginia Stiles, Lois Evelyn Norris, Elizabeth Graham, Thelma Meredith, Mary Jenise House, Marilyn Stinson and Peggy Wentworth.
He is survived by: his children, John Douglas Lewis, Jr., Robert Floyd Lewis (Melancholy), David Lewis (MaeKayla) and Jennifer Zadow; his grandchildren, John Douglas Lewis III, Jesse Lewis, Tyler Lewis, Eiley Lewis, Kaydence Clark and Katherine King; and his siblings, James Lewis (Brenda), Ricky House, Patty Goodman and Debbie Raisor (Floyd). He is also survived by several nieces and nephews.
