He is survived by: his wife Pam Heath of Bowling Green, KY; his children, Cody Heath (Jaclyn) of Alvaton, KY, Leslie Sledge (Jeff) of Smiths Grove, KY and Holly Henry (David) of Bowling Green, KY; his brother Larry Heath (Pam) of Scottsville, KY; and his grandchildren, Ella, Heath Sledge, Logan, Luke, Zoey Henry, Elsie, Milly and Alice Heath.
