He was predeceased by: his parents, Clyde Perkins and Della Mae Perkins; and his sister Anna Rhea Millard. He is survived by: his wife Martha Delores Perkins; his children, Jackie Perkins (Patti) and Louise Henderson (Dale); his grandchildren, Dr. Jack Perkins (Tiffany), James Perkins (Teresa), Jenna Payton (Kaleb), Audrey Shedivy (Dan), Whitney Henry (Corey) and Claire Butler (David); and his great grandchildren, Ricky, Alissa, Nolan, Jay, Audrey, Lauren, Taylor, Toby, Greta, Emerson and Colby.
