He is survived by: his parents, Jerry David and Peggy Hendricks Futrell of Cadiz, KY; his brothers, Brian Heath and Leslie Futrell (Fletcher) of Cadiz, KY; his nieces, EllaRhea and EmmaLee Futrell of Cadiz, KY; and his aunts and uncles, Melba Futrell of Cadiz, KY, Johnny Adams of Cadiz, KY, Benny Alcaraz of Ely, NV, W. C. Hendricks of Cadiz, KY, Vernon and Suellen Hendricks of Cadiz, KY.
