He was predeceased by: his parents, Jeff Thomas and Mary Helen Lively. He is survived by: his wife Mary C. Lively; his sons, James Lively (Deana) and Jeff Lively; and his sister Valerie Edgerly (Paul). He is also survived by seven grandchildren.
